Summary

Consistency fixes identified in the post-merge review:

  • Fix $AGENT\_HOME → $AGENT_HOME in kubectl-karen, null-pointer-nancy, and regression-regina AGENTS.md files — escaped underscores render incorrectly in some contexts
  • Standardize section dividers in Gandalf and Hugh SOUL.md: *** → --- to match all other agents
  • Update TOOLS.md and OPERATIONS.md to replace get-github-token.sh references with the github-app-token skill
